Planning Process - Personnel Request

Personnel Request

 

Launch your web browser and navigate to https://slam.teamdynamix.com.

When the site loads, click on “Client Portal”

When the site launches, confirm you are logged in by looking to the right of the “Search the client portal” box. It should say your name, if it reads “Sign in”, click it and follow the steps.

Once signed in, click on “Services”

When the “Service Catalog” appears, scroll down and click on “Planning Process”

Next, click on “Personnel Request”

When the “Personnel Request” page appears, click on “Request Service”

After clicking “Request Service”, you will be presented with the following form.

After you have submitted your request, you can check the status by navigating to the “Client Portal” then click on “Services”

Then click on “Ticket Requests”

When the page opens, scroll down to see your requests. You can click on the “Title” of your requests to get more details.

 

Once the request has gone through all the necessary reviews, if approved, you’ll receive the following email.

If at any point in the process your request is denied, you will receive the following email.
